Connective tissue disease-induced gastrointestinal vasculitis: a clinical analysis of 14 cases

چکیده
Investigate the clinical characteristics of connective tissue disease (CTD) patients with gastrointestinal vasculitis to improve diagnosis and treatment. Retrospective analysis of the clinical presentation, examination, testing, treatment and prognosis from 14 CTD patients with gastrointestinal vasculitis during a 12-years period. All 14 patients had clinical manifestations of gastrointestinal involvement and (or) symptoms. The most common clinical presentation was abdominal pain, followed by hemafecia. Laboratory tests showed positive fecal occult blood in 9 patients. Seven cases underwent endoscopy and nonsurgical medical treatment. Seven patients underwent surgical exploration, and were diagnosed intestinal necrosis in 3 cases, gastrointestinal perforation in 5 cases, peritonitis in 7 cases, intestinal obstruction in 3 cases and gastrointestinal ulcer in 7 cases. Histopathological examination identified significant neointimal hyperplasia, obliteration, and fibrinoid necrosis in the small blood vessels, or thrombosis. Six patients died, 4 during the surgery and 2 cases died during the nonsurgical treatment. CTD patients with gastrointestinal vasculitis showed unspecific symptoms, but had high mortality rate. Early diagnosis and prompt treatment can improve the prognosis. Positive fecal occult blood test can be considered as a warning sign, digestive endoscopy is the most direct method for early diagnosis.

A Case and Literature Review of Normal Pressure Hydrocephalus in Mixed Connective Tissue Disease
Normal Pressure hydrocephalus (NPH) is characterized by gait apraxia, urinary incontinence, and dementia. Mixed connective tissue disease (MCTD) is an autoimmune connective tissue disease that has never been reported to cause NPH. Our patient was a 67-year man with a one-year history of gradual worsening gait and balance, urinary urgency with urge incontinence and decreased short-term memory. P...

Study objectives: To demonstrate that pulmonary capillaritis and diffuse alveolar hemorrhage (DAH) occur and are isolated to the lung and therefore not part of systemic vasculitis at the time of the DAH episode in rheumatoid arthritis (RA) and mixed connective tissue disease (MCTD). Design: Lung biopsy specimens from patients with DAH were reviewed and those with the histologic features of pulm...

Vasculitis is an inflammatory disease of blood vessels that can affect all types of vessels in any organ of the body, including the vessels of the gastrointestinal (GI) tract. It may be secondary to systemic disorders, such as connective tissue disease (e.g., systemic lupus erythematosus, and rheumatoid arthritis), infection (e.g., bacterial endocarditis), malignancy, or drugs. Primary localize...
متن کاملPauci-Immune Crescentic Glomerulonephritis in Connective Tissue Disease
Pauci-immune crescentic glomerulonephritis is commonly seen in ANCA-associated vasculitis but it is rarely seen during the course of other connective tissue diseases like lupus or Sjogren's syndrome or MCTD. We report 3 cases of pauci-immune crescentic glomerulonephritis in patients with connective tissue disease other than vasculitis. We reviewed literature and made summary of previously repor...
متن کاملB cell Therapy to Treat an Axonal Neuropathy in Mixed Connective Tissue Disease
The B cell is a vital contributor to humoral immunity. The B cell-specific antigen CD20 is expressed during B cell development, starting at the pre-B cell level and persists through B cell differentiation, but is lost during terminal differentiation to plasma cells. Rituximab is a monoclonal antibody that destroys both normal and malignant B cells that have CD20 on their surfaces and is ther...
